Request to Modify an IEP, 504 Plan, or Other Intervention Plan
If for some reason you believe your child’s behavior plan in school needs to be modified, you always have the right to request that the team, which includes you, meets to discuss your concerns. It’s recommended that all requests are submitted in writing and that you save all communications in one place in case you ever need to show documentation. It is important to document all communications in writing, even if only to summarize and put on paper the things that were discussed verbally.
Some of you may have formal Individualized Education Plans (IEPs), some may have 504 Accommodation Plans (504s), and some may not have such formal plans, but are working with the school on details to help your child, nonetheless. Below is a template of a letter. There are many free websites with a variety of letter templates. This is one popular website: Wright’s Law. Feel free to edit the sample below as you please to suit your situation.

I am concerned about (state your reasons, but don’t go into detail about the specific changes you want to make—save those for the meeting). I would also like to have (names of specialists or other staff) attend. I am hopeful that an exchange of ideas with all of us will be valuable. I can arrange to meet with you and the other team members on (days) between (give a range of time, such as between ___and ___). Alternatively, please let me know what time would be best for you if the suggested times are not doable.
